Comprehensive Care Management (CCM) is a not-for-profit New York State Managed Long Term Care organization. Founded in 1985 to provide home healthcare services to the elderly, disabled and chronically ill in the New York metropolitan area, CCM today is recognized as an innovator in the managed care field. Its Program of All-Inclusive Care for the Elderly (PACE), which enrolled its first member in 1992, is today the largest such program in the nation.
One of the distinguishing features of CCMs care model is our team-based approach. At our 12 PACE Centers and 5 alternative care settings, serving the New York Metropolitan region, our team of healthcare professionals work side by side to provide knowledgeable, attentive and culturally sensitive care to our members. Indeed, we are fluent in both the languages and customs of our members.
Whether were providing care in a members home or in one of our day centers, our goal is always the same: to help our members enjoy the highest possible quality of life while living safely in their homes and communities.
